Wychavon District Health Collaborative (WDHC) is a group of people and organisations working together at a local level to meet the needs of the communities they serve.

We want to help people to remain well for longer. We can do this by making sure all communities have access to support and services which help us to be well and prevent an increase in the severity of their needs.

Wychavon is the largest district within Worcestershire.

As we have many differing communities across a large geographic area and with the need to remain informed and affect change at a local level, we have established three place-based working groups.

These groups are made up of local organisations and Primary Care Networks (groups of GP Practices also called PCNS), who really understand their communities.

Membership of the WDHC

Groups involved in the WHDC include:

  • Wychavon District Council
  • Groups of GP Practices (known as Primary Care Networks)
  • Voluntary, community and charity organisations
  • Schools and colleges
  • NHS Trusts
  • Worcestershire County Council
  • Police
  • Fire

WDHC priorities

We have five priorities. These are based on data about the local health and wellbeing needs of people living in the district. These are listed in the table below:

Tackling loneliness and social isolation

In particular there will be a focus on preventing Type 2 Diabetes and early-onset dementia.

Providing the best start in life during maternity and children's health

There will be an emphasis on mental health and improving health outcomes which tackle childhood obesity.

Targeted engagement where inequality and/or deprivation is linked to health outcomes

Key benefits will include improved access to health checks and screening for prevention and early diagnosis of illness. 

Promoting healthy lifestyles

There will be a focus on fraility prevention, enabling independence and the cost of living.

Improve mental health and wellbeing

There will be a key focus on access to services and digital inclusion.

Community and Living

Community - Funding - Apply

This section offers a wide range of advice and support for community groups and voluntary organisations across Wychavon. You’ll find details on funding opportunities, the WYCH Rewards scheme is designed to reduce carbon emissions and tackle climate change and resources on social mobility. There’s also a map of community facilities across the district, along with information on available transport options.

Support for health and wellbeing is included too, covering mental health, suicide prevention, the Wychavon District Health Collaborative, the deaf and hearing loss group, the ‘Defy Dementia’ campaign, and our youth vaping initiative aimed at reducing the risks of vaping among young people.

Worcestershire on Demand (WoD) is a transport service that works differently from normal bus routes. It helps residents and visitors get to important places nearby, like shops, schools, or doctors, within a set area. WoD also takes people to places outside that area, making it easier for everyone to get around.

Community Funding and Support

We offer a range of advice, funding and other support to community groups and voluntary organisations across Wychavon to help them respond to local priorities and needs.

Communities Officers

Our three Communities Officers work closely with communities and a wide range of organisations across Wychavon including the three volunteer centres, parish and town councils, food banks and community groups. They support a variety of projects that will make the district a better place to live as well as supporting communities to respond to local priorities. We will be adding more details about how you can contact them and the support they can provide soon.
